Membership program is a dental membership plan designed for those without dental insurance, offering 12 months of eligibility for an enrollment fee of $299. The plan does not have treatment exclusions or waiting periods and cannot be used with dental insurance. It auto-renews annually, exclusive to Lake Jackson Dental Care in Lake Jackson, TX. Members must clear their accounts before enrollment and cannot retroactively use benefits. Unused benefits do not carry over or refund.

The plan is non-refundable and non-transferable. Members must pay treatment fees upfront to maintain discounts, with different discounts for Care Credit or Lending Point users. Ortho treatment is not eligible for the 15% treatment discount, which cannot be combined with other offers. Dental service fees may change, with estimates valid for 90 days.

Active participation in the DDS hygiene program is required for benefits, including scheduling recommended periodontal therapy and next preventative appointments.

$299 – Annual membership – One time payment.

Membership starts on the day of enrollment. Automatically renews, unless’ notice is given at least 1 week before the end of current term.

Membership includes:

• 1 Complete Exam (done for new patient/once every 2-3 years for existing patient), 1 Periodic exam (done every 6 months for existing patient).
• X-rays required for 1 complete exam and 1 periodic exam.
• In office fluoride application 2 times a year.
• I Limited exam1 Peri-apical X-ray, and 1 BW ( Pano at 15% discount if required).
• 15 % off on all other services
